LENGTHENING PENOPLASTY
What are the results in terms of lengthening, at rest and during erection?
The average increase varies between 2 and 4cm in most cases, visible both at rest and at to a slightly lesser degree during erection
What determines the variability of the results?
The consistency and the dimensions of the suspensory ligament, which vary from person to person
Are the results stable over time?
The specific suture (periosteal-fascial) used in the lengthening penoplasty enables the two parts of the sectioned suspensory ligament to be flexed inwards to a great degree. This measure impedes the operated tissues from shrinking back (post-surgical scar retraction) and annulling all or part of the result obtained
Besides the elongation of the penis, are there any other visible changes?
Practically not, because with the periosteal fascial suture, the normal anatomy of the penis is reconstituted and as a result even the angle of erection does not vary significantly
Does the penoplasty leave any visible marks?
No. The incision is of a small dimension, it is made using plastic surgery methods and is performed in the supra-pubic area where it will be covered by the pubic hair and will therefore be invisible
When and why is an extensor used?
Despite not being mandatory, from the thirtieth day after the operation and for three months thereafter, the use of an extensor or vacuum, devices which put the penis under controlled and progressive traction, is recommended. The use of these devices prevents scar retraction and consolidates and improves the result obtained
Is it possible to associate a lengthening penoplasty with other operations in a single surgical
session?
Yes, it is certainly possible to associate a lengthening penoplasty with an enlargement penoplasty (girth enhancement) and other operations indicated in the
table.
Is it possible to increase the volume of the glans?
Yes, both during as an outpatient and using local anesthetic. The procedure consists in the injection of hyaluronic acid into the superficial layer of the skin of the
glans.

What are the results in terms of enlargement?
The average increase in circumference is of 30%
Is the fat reabsorbed?
Thanks to the purification of the fat the percentage of reabsorption generally does not exceed 20-30%. In order to obviate this physiological reabsorption, a slightly higher than necessary quantity of purified fat is implanted during the operation
Is the consistency of the penis altered?
Initially there will be an increase of mushiness of the shaft which becomes less evident as time goes by. After a few weeks the reparative processes and formation of new capillaries will stabilize the fat unifying it with the corpora beneath determining a stable increase in consistency at rest and during erection, which will eventually seem similar to that prior to the operation
Are the results stable over time?
An absorption of more than 30% of the fat implanted is rarely observed; if this occurs, the phenomenon is linked to individual characteristics such as the metabolism and lifestyle. Considering the extreme simplicity of the method, any reabsorption of can easily be compensated definitively by implanting more fat
Does the penoplasty leave any visible marks?
No. The incisions for the cannula are millimetric and become invisible in about 2 weeks
To which surgical operation can an enlargement penoplasty never be associated?
It is not possible to associate an enlargement penoplasty with circumcision, the surgery for Induratio Penis Plastica and recurvatuum penis. It is, however, possible to perform it at the same time as other operations especially the lengthening penoplasty, as shown in the section entitled
"Combinations".
Why is the volume of the glans not increased with fat?
Because the glans is made up of spongy tissue whose volume increases physiologically when it is filled with blood. Any substance implanted into it would reduce the surface available for the blood, resulting in the altered expansion of the tissue. Hyluronic acid can be injected into the superficial layer of the skin at least 30 days before or after the enlargement
penoplasty.
The implant causes an evident and uniform increase in volume. The hyaluronic acid is reabsorbed gradually and progressively in the region of 12-18 months, after which the procedure can be repeated in the doctors rooms.
Prof. Alessandro Littara was born in Pisa on 15 April 1964. He is an authority in male genital aesthetic surgery thanks to his pioneering work in penoplasty, a technique he has performed since the 90’s and which he has continuously modified, improved and perfected during his personal experience with thousands of cases from all around the world.
Training (1991) at the Centre for Andrology of the University of Pisa (directed by Professor G.F. Menchini Fabris), where he currently works as an external collaborator. He also specialized as an Andrology module operator and in Medical Sexology, at the same centre.
Long collaboration with the Istituto di Chirurgia Generale II - Unità Operativa di Endocrinochirurgia (directed by Prof. P. Miccoli), of the Università degli Studi di Pisa, with respect to surgical practice.
Student of Uro-andrological surgery as taught by Prof. E. Belgrano (director of the Urology clinic of the University of Trieste) and Prof. F. Carmignani (director of the Urology clinic of the University of Genoa) for more than five years, staying up-to-date with Avant-guard Uro-andrological and gynaecological urology surgical techniques.

Radio 24 collaborator with regards to medical and surgical andrology and sexology. He collaborates with national newspapers, weeklys and monthlies with wide distribution and also with national and regional television programmes. He conceived and conducted a radio transmission dealing with sexology (SessoOS) at a local Tuscan radio station for two years. He participates as a subject matter expert in the transmission "Istruzioni per l'uso" presented by Emanuela Falcetti on Radio 1 and Radio 3.
Founder and head of the "Centro di Medicina Sessuale" of Milan, where all pathologies of the sexual and reproductive sphere for both males and females are treated. He manages a multi-disciplinary team made up of andrologists, gynaecologists, assisted reproduction specialists, psycho-sexologists, psychiatrists, plastic-cosmetic surgeons and lawyers.
Training and certification to perform procedures of female genital laser surgery at the Laser Vaginal Rejuvenation Centre of Beverly Hills, directed by Dr. David Matlock. Surgeon affiliated with and certified to perform DLV (Designer Laser Vaginoplasty), LVR (Laser Vaginal Rejuvenation) and G-Spot Amplification procedures, by the said centre.
He currently works as a freelance professional and consults in Milan, the main seat, and periodically in Rome. He collaborates with centres specialising in Infertility therapy for couples, and is a contract Professor of Uro-genital Reconstructive surgery at the University of Pisa.
